Medical Respite Care
TCG’s current medical outreach/respite program will move to the Roots of Community Campus to improve coordination of care, social exposure for recovering individuals, and offer more access to respite care to those in need in our community.
Respite Care Roots of Community
Will offer 20 respite beds - increasing the capacity of respite care in our community by 200%
An onsite exam space equipped with Tele-health capabilities to improve patient care and reduce waiting room time and the need for transportation for patients.
We will NOT require ID and will work with the individual to secure documents needed as they recover.
Will strive to not discharge to the streets but will work with the available resources in our community to find a sheltering solution for each individual we care for.
Provides a cost savings to local hospitals and seeks to reduce ED visits, reduce street discharges from medical care, and reduce the possible spread of communal viruses.
